BiigCountryy- I am amazed how well BJJ put my thoughts in words. Well then again not really. Kuddo's BJJ. But there was a whole lot in this story. So hats off, I echo BJJ.

Wow. You and your family been through a lot and those events sounds really crazy and weird. I believe the old lady in the white dress is not harmful but I think she wants to say something or show you something. I also think that she probably can't find her way to the other side. That dark figure sounds like it likes to watch you guys and cause trouble. To me it sounds like there's at least 3-4 spirits you and your family has seen. Whenever you see or hear a spirit you must not show fear they can use your fear as energy and manifest themselves. I know it sounds hard to do but try your best not to show fear. If it was me I would also like to keep my dogs with me too. I think the land you live on right now could have some sort of history behind it, there's something suspicious about it. If you can try to find out about the history of the house and the neighborhood that you live in. I think you might need to get the house and your family blessed. I hope this helps a bit.

I am only going to address a few things here and leave the rest to others.
1) "My grandmother being the worst. She was always very nice, but now I have never met a more hateful person." Unfortunately, that can happen with illness and age. It's unpleasant for all parties. Being sick can change you, especially with certain treatments. She may also be experiencing negative side effects of treatments and even the starting stages of dementia, which can cause even the sweetest person to become someone who is very hateful. This, sadly, is not paranormal, but the sign of an ill person.
2) "We used to have a large clock that was hung up in the kitchen. One day the clock stopped. We didn't bother with it for awhile. A week after it stopped, pope John Paul died. When we decided to fix the clock a few weeks later. We realized the clock stopped on the exact second of when the pope died. That's a pretty strange coincidence." - Strange, yes, but I do believe that to be a coincidence. That is not necessary a sign of anything paranormal. Strange coincidences just happen sometimes.
3) Be careful about trying to draw a lot of meaning out of dreams. Dreams are your subconscious. If you are scared or think something evil is in your house, your dreams with mimic that fear and create nightmares. Having dreams about being possessed does not mean something is actually trying to possess you. If you are experiencing a lot of activity, it is only natural that you would dream about it.
I am just trying to separate paranormal encounters from those that are explainable. I hope you are doing well.
Since you mentioned the Pope, I will guess that you are Catholic. Have you not thought about getting your house blessed by a Priest?

I'll be very honest in my comment and opinions, and that will very likely be annoying for you. I hope not, but I tend to be blunt.
My first impression of your story is that you may be experiencing a combination of poltergeist activity as well as some typical haunting activity. The old lady you describe sounds like a garden variety haunt, not threatening and ultimately no big deal. She may need help crossing, if she's willing to go. We'll get to that later. I'm not sure where I stand on the male voice, but I'm inclined to think he isn't truly a problem either.
Poltergeist activity isn't a true haunt, it isn't in any way intelligent or aware of its surroundings. Poltergeist activity is just a manifestation of energy, and it seems to come from living humans. People who are under stress can sometimes build up too much tension, too much energy, and that energy manifests itself as a poltergeist. Poltergeists can be as simple as hearing sounds, or as intense as moving objects or causing mild injuries, such as scratches.
Your story makes it sound as though the more intense activity is occurring during times of heightened tension in your family, such as your grandmother's diagnosis and your parent's fighting. I think the more chaotic stuff you've described is very likely to be poltergeist energy instead of an independent entity.
To be honest, every time the 3 am and 3 scratches thing comes up, I have very difficult time taking the claims seriously. Those are just so pop culture stereotypical that I almost suspect people throw it in, whether or not it happened, to add credibility to their claims. These days everyone thinks its not legit until 3 am comes into play. I told you I'd probably offend you.
Assuming honesty on your part concerning the 3s, my thoughts are A: your subconscious has associated 3 am with "scary time" and is causing you to wake up. B: the subconscious association with 3s influenced the poltergeist energy to make 3 scratches.
I also think if you guys watch a lot of paranormal shows, The Haunted, Ghost Adventures, etc, its a good idea to stop. Some of those shows really hype the 3 stuff, and if a person expects something to happen, more often than not it will. Especially if I'm right concerning the poltergeist theory, don't give your subconscious ammo.
It's not surprising that your family is experiencing mood swings and that your grandmother is becoming temperamental. Cancer, to be blunt, is a female dog. It causes physical as well as emotional suffering to the sick individual, and the tension on the family, its a waking nightmare. I've seen what cancer does to a family, and my suggestion on the mood swings, just be patient. You guys need each other, now more than ever. Try to be forgiving, try to hold on to one another and be supportive. Cancer attacks entire families, and the treatment for the disease is almost as bad as the disease itself. Please, just stick together.
My final suggestion and I'll shut up. Since this is very intense and has been gong on so long, I would recommend calling in a paranormal team to come to your home. Do your homework on any team you consider, check references, make sure they are legit. When you find a reliable team, call them. Be honest, be open, let them know everything. They can better assess what's happening than we can online, and they can help you find solutions that will help your family. The ghosts, the old lady and the man, a good team will have resources to help those ghosts find their way to the other side.

However, you are likely to wake up at 3 a.m. Out of your bodies need for preservation. Your body is at it's slowest during that time and if your body slows down to far, your body will wake itself up. It's a natural process.
BadJuuJuu brought up quite a few very, very good points that are likely to help quite a bit.
Be kind to your grandmother, as well. No matter how mean she may seem, she is very sick and likely afraid. Be kind and be there for her. She needs her family.
